We are happy to register new patients who live within our practice boundary area. In order to undertake the registration process we require you to complete a registration form and questionnaire. Please complete an Under 5 Questionnaire for any children aged under 5 years. These forms can be downloaded below or collected from the surgery reception.
Please be thorough and complete all sections in the registration form, as unfortunately we are unable to process incomplete forms, and this may delay your registration. Your application will be processed within one to two working days.
In order to establish NHS entitlement we need to see some form of identification in person before you can be registered. Please bring along with you two items of identification, one which must include details of your current address.
The completed registration forms must be handed in between the hours of 9.00 am - 5.00 pm, Monday, Tuesday, Thursday, Friday and 8am - 5.45pm on a Wednesday.

A Patient Guide to GP Services
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
